As a marketing research project I am trying to determine who pays for screen readers, users, family, friends, employers, service clubs, or governmental agencies. I suspect that governmental agencies pay for the overwhelming number of screen readers but I've never seen quantitative evidence of this.
I would appreciate very much your completing the following questionnaire. It's short with four radio controls and three text boxes, and completely anonymous. If you choose to participate then please complete the form only once. The data collected will be meaningless otherwise.

What is your primary screen reader:
Connect Outloud
HAL
JAWS
Outspoken
Simply Talker
Window Eyes
Window Bridge
Other
If other then please enter its name here:

Who payed for your primary screen reader:
I did
My Family did
My Friends did
My Employer did
A Service Club did
A Governmental Agency did
I'm borrowing it
Someone gave it to me
Other
If other then please describe here:

If you have a secondary Screen Reader then please continue. Otherwise skip ahead to the "What country are you in" question which is followed by a place to enter comments, then the Submit button.
